SPORTS PHYSICAL DAY (May 10, 2023 @ DWHS)
¨ Physicals will be offered on Wednesday, May 10, 2023, starting at 9:00 am at DWHS for students who will be in grades 6-12 during the 2022-2023 school year. Students who are currently in the 5th & 6th grades will be bussed to the HS to get physicals completed. Sixth-grade participation is TBD for the 24-25 school year.
¨ Amberwell will provide physicals. Each physical is $25 payable by cash or check to Amberwell (no insurance accepted).
¨ JH & HS students who plan to be involved in athletics MUST have a physical, concussion, & eligibility form on file prior to the first day of practice for the 23-24 school year. The physical MUST be dated after 5/1/23 to count for the 23-24 school year.
¨ Complete the appropriate forms (physical, eligibility, & concussion forms) and return to DWHS (with payment) by May 9, 2023. Physicals will take place during the school day on May 10.
